Procedure for requesting a disbursement|
Submit the disbursement form via fax or mail to The Executive Director at the address at the bottom of this page. You may download disbursement guidelines here.
The original or copies of receipts are required for all purchases.
All checks will be made out to the designated vendor(s).
For a credit card purchase, submit the bill along with a copy of the receipt and complete the Payment Request Form describing each purchase.
Call or email the Executive Director in cases where there are questions regarding the appropriateness of the request. A list of expenses that may be paid for using trust funds may be found by following this link.
You will receive a written response with an explanation if the request is denied.
